fix: 哨兵注释 + 保守 fallback，杜绝 --uninstall 数据丢失

v1.2.0 的 cleanBootstrapSection 用 indexOf('\n# ') 找段尾，当 superpowers
段在文件开头、后面只有 ## 二级标题时，会错误地把段尾扩展到文件末尾，
导致用户在 superpowers 段之后写的二级标题内容被一并删除。

修复：
- 安装时给 4 处追加型 bootstrap（CLAUDE.md / GEMINI.md / HERMES.md /
  CONVENTIONS.md）的内容用 <!-- superpowers-zh:begin/end --> 哨兵包裹
- 卸载优先按哨兵精确切除；找不到时 fallback 到标题 marker；
  再不行用末尾固定句子兜底；都不行就跳过 + 警告，绝不删用户数据
- isHomeDir 在 homedir() 返回空字符串时返回 false（避免误判 cwd 是 home）
