Samuel Serapion d99afa913f Fix some compiler warnings.

Implement InitializeCriticalSectionEx
Stub TermsrvAppInstallMode and SetTermsrvAppInstallMode.
Lazy port of BasepInitializeContext.
Lazy ports of thread and fiber startup code, probably will not work.

Change the logic and make the AnswerInPublic Command option opt-out instead of opt-in. Now everything except !help is allowed in public again (due to popular demand).
