Subject: [PATCH] update README: add 4 new providers, simplify install section

- Add OpenAI Codex (IDE), Anthropic, Cloudflare AI, HuggingFace (API)
- Update provider count: 29 → 32 (8 IDE + 24 API)
- Update provider tables with accurate free tier info
- Merge install & quick start into one section
- macOS: direct download link + xattr quarantine fix
- Remove redundant --setup/--test commands from quick start
