Luis Pater e5b5a1cfca fix(kimi): reorder interleaved tool outputs in responses input 
- Defer intervening non-tool messages until all batch tool outputs are emitted to keep outputs contiguous
- Apply responses input normalization across standard and streaming executor execution paths
