Luis Pater 519d590e83 feat(streaming): support send-keepalive-before-ft option ...

- Add `send-keepalive-before-ft` configuration to allow sending SSE heartbeats before the first token arrives.
- Support pre-first-token keep-alives and handle early failure events when SSE headers are already committed.
