Luis Pater 9e79552fb7 fix(codex): hold response.metadata during stream bootstrap buffering 
- Add response.metadata to isCodexBootstrapBufferableEvent so prompt-moderation metadata frames do not prematurely release uncommitted Codex streams before server_is_overloaded rejections.
- Add unit tests covering response.metadata classification and bootstrap overload failover across SSE and WebSocket transports.
