Luis Pater 97dd9eec62 fix(openai): report error on truncated stream missing finish_reason ...

- Track `finish_reason` presence across initial and streaming response chunks
- Return a terminal bad gateway error instead of `[DONE]` when upstream stream closes prematurely without a finish reason
